The registration of birth and death is essential in each and every Indian state. The certificates are needed to get different services from the central or state government. In Haryana, now the government had made it pretty easy to apply for a birth or death certificate. The process is simple but had to be followed cautiously to get the certificate quickly. The details of the application process are mentioned below.
Things to Keep in Mind:
- The death or birth of a certain person must be registered with 21 days time.
- After 30 days, you have to pay a late fine of INR 2 and a penalty of INR 5.
- After 60 days, the late fine will be increased to INR 5 and the penalty will be INR 10.
- If a year passes by, then the late fine will be INR 10 and the penalty will be INR 25.
- To get the certificate free, you must apply within 21 days.
Application Process For Birth/Death Certificate in Haryana:
- For the application of birth or death certificate, you have to visit the website http://haryanahealth.nic.in/menudesc.aspx?page=88.
- You will find both the applications on this website.
- Choose the appropriate application which you want to do and download it. Take print out on A4 sized paper.
- Fill up the form with all correct details before submission as any wrong detail may end up in canceling your application.
- Now, attach the form with all required documents and then submit the form to the registrar office.
- Before issuing the certificate a complete verification will be done and then only the certificate will be issued to you.

Documents Needed For Birth Certificate:
- Birth Certificate of both the parents.
- Marriage Certificate of Parents
- Hospital issued proof of birth
- Identity proof of parents
- If birth took place at home, an affidavit with the birth details.
Documents Needed For Death Certificate:
- Some information related to the deceased person like name, religion, gender, nationality, etc.
- Date of death
- Address of the person
- Father’s or Husband’s name of the deceased person
- If the death occurred at the police station, jail, or hospital, a report from the authority to prove that.
- Post-mortem report.
